In order to get mail order contact lenses you will undoubtedly need to fill out a form with you name, mailing address and payment method. This form may also include boxes for you to fill in your prescription information; however, you will still need to supply at the very least a photocopy of your prescription with your doctor’s signature. Some companies will only accept an original prescription; they will not accept a photocopy, so be sure to ask about that.
